Koepka, 28, struck the fan in the face on the short, par-4 sixth hole.
Rich Beem, on-course commentator for Sky Sports, said Koepka was clearly shaken-up as he missed a number of putts. Beem added the supporter had suffered a nasty injury after the ball hit her just above the eye.
Koepka, who partnered Tony Finau, went on to defeat Justin Rose and Jon Rahm on the final hole to land a narrow 1 up victory.
The Americans have taken an early 3-1 lead into the afternoon foursomes.
